Power of Attorney
Health Care
A legal designation selecting a person or agency to serve as alternative decision maker if or when you are not able to make your own health care decisions. To be activated, in the state of WI, this designation requires two Doctors to conclude you are no longer able to make your own health care decisions. Activation is usually due to a decline in cognitive ability due to various medical reasons. This document can be created with the help of an attorney or using State of WI forms. This designation cannot be made after one has been found incapacitated and in need of an alternative decision maker. If a POA of Health Care is not designated prior to incapacitation, Guardianship may have to be pursued to allow someone to make your medical decisions for you.
If your designees die or refuse to serve – Guardianship may need to be pursued. When you select Golden Care Services, there will be no reason for your designation not to be honored. We will be there when you need us, making Guardianship highly unlikely.
Power of Attorney
Finance
A legal designation naming a financial agent to manage finances, assets, or pay bills when an individual is no longer able to manage their own finances, or when they wish for another person/agency to take over management of finances in a supportive manner.
This document can be created with the help of an attorney or using State of WI forms. This designation cannot be made after one has been found incapacitated and in need of an alternative decision maker. If POA of Finance is not done prior to the loss of ability, for a person, to make their own financial decisions, Guardianship may have to be pursued to allow someone to make your financial decisions.
If your designees die or refuses to serve – Guardianship may need to be pursued. When you select Golden Care Services, there will be no reason for your decision not to be honored. We will still be there when you need us.
Trustee
A legal designation allowing a person or agency to manage the day-to-day actions of a legal trust set up by a lawyer. Trustees are responsible for following the designations of a trust.
If your selected Trustee dies or refuses to serve – Legal steps need to be pursued. When you select Golden Care Services, there will be no reason to worry about support and implementation of your decisions. We will be there when you need us.
Personal Representative
Personal Representative is the designation of an individual or agency to settle your estate after you pass. Often the Personal Representative is a family member or friend. Frequently, the settlement of an estate is difficult on the family and causes unnecessary challenges in relationships. When choosing an agency to be your Personal Representative, there is no conflict of interest in following your final wishes.
